To respond to your query, we need your e-mail address, phone number and date of birth. (This is for response purposes only to obtain additional information and to verify that the correct member information is being reviewed. MAMSI will not release your information to any third party.) We will respond to most routine questions via return e-mail within 24 hours.

If you are requesting materials by mail, please enter the address where the materials should be sent. No post office box numbers, please. You should receive most materials within 5 to seven business days.
