	Optimum Choice, Inc. (Optimum Choice), a MAMSI health plan, offers Anne Arundel County Government/Library/Community College employees an HMO plan.

With this plan, you select a Primary Care Physician (PCP). You may use your PCP to coordinate all your health care. When your PCP coordinates your care and gives you a referral to see a Specialist, you pay a minimum copayment for your visit to the Specialist. You may see a participating gynecologist without a referral from your PCP. Optimum Choice also covers an annual eye refraction exam from a participating provider without a referral.

You may change your PCP on this Web site or by calling member services. If the Member Services Department receives PCP change information on or before the 20th of the month, you can start using your new PCP at the beginning of the next month.
